Post-Hospital & Specialized Support

A Smoother Transition Back to Life at Home

The days after hospitalization or rehabilitation can be demanding. New routines, reduced strength, household responsibilities, and family concerns can make the return home feel overwhelming. ArborCircle provides practical, non-medical support while the client recovers and adjusts.

Practical Help at Home

Transitional Support May Include

Every service plan is built around the individual’s needs, the home environment, and the recovery goals your family and care providers have identified.

  • Personal care and dressing assistance
  • Meal preparation and hydration support
  • Mobility and transfer assistance within the service plan
  • Light housekeeping and laundry
  • Appointment accompaniment and errands
  • Reminders for self-administered medications and established routines
  • Observation and reporting of changes or concerns
  • Family respite and communication
Beyond Recovery

Support for Ongoing Conditions

Non-medical care may also support adults living with Parkinson’s disease, mobility limitations, chronic illness, physical disability, or other conditions that affect daily living.

Whatever the diagnosis, the focus stays on the person’s routines, preferences, and goals—so support feels continuous rather than clinical.

Working Alongside Clinical Providers

When a client also receives services from home health, hospice, physicians, therapists, or other licensed providers, ArborCircle follows its non-medical scope and coordinates communication as authorized. Caregivers follow the ArborCircle service plan and do not perform clinical tasks delegated informally by clients or families.
